As a Hospital Liaison, you will be the primary contact between our company and hospitals, ensuring smooth transitions and high-quality care for our patients.   • Establish and maintain positive relationships with hospitals and their staff to facilitate patient care and ensure a seamless transition from hospital to homecare setting
  • Serve as the primary point of contact for hospitals, communicating effectively and efficiently with various departments and personnel
  • Coordinate with hospital discharge planners and social workers to identify potential patients in need of home oxygen therapy and other medical equipment
  • Conduct thorough assessments of patients' needs and educate them and their families on the services provided by Lincare Holdings
  • Collaborate with healthcare professionals to develop individualized care plans for patients and ensure their needs are met
  • Monitor and track patients' progress and provide ongoing support and guidance as needed
  • Conduct regular follow-up with hospitals and patients to ensure satisfaction and address any concerns or issues that may arise
  • Stay up-to-date on industry trends, regulations, and best practices to provide the highest level of service and care to patients
  • Participate in marketing and networking efforts to promote Lincare Holdings and build relationships with hospitals and other healthcare providers
  • Adhere to all company policies and procedures, as well as state and federal regulations, to maintain compliance and promote a safe and ethical work environment.

Job Qualifications
