  1. Build and maintain strong relationships with hospital partners to promote collaboration and communication.
  2. Serve as a point of contact for hospital staff, patients, and their families during the transition from hospital to home care.
  3. Educate hospital staff on the services and support provided by Lincare Holdings.
  4. Conduct thorough assessments of patient needs and develop individualized care plans.
  5. Coordinate with hospital discharge planners to ensure a smooth transition for patients.
  6. Communicate effectively with patients and their families to address any questions or concerns.
  7. Collaborate with other members of the Lincare team to provide seamless and comprehensive care for patients.
  8. Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  9. Stay up-to-date on industry trends and changes in healthcare policies and regulations.
  10. Maintain accurate and detailed records of patient interactions and progress.
  11. Participate in marketing and outreach efforts to promote Lincare services to hospitals and other healthcare facilities.
  12. Adhere to all company policies and procedures, as well as ethical and legal standards.
  13. Continuously strive to improve patient satisfaction and outcomes.
  14. Represent Lincare in a professional and compassionate manner at all times.

Job Qualifications
  • Knowledge Of Hospital Operations: A Qualified Hospital Liaison Should Have A Comprehensive Understanding Of Hospital Operations, Including Patient Care Processes, Discharge Planning Procedures, And Insurance Requirements.

  • Excellent Communication Skills: As A Hospital Liaison, One Must Have Excellent Communication Skills To Effectively Communicate With Hospital Staff, Patients, And Their Families. This Includes Strong Verbal And Written Communication, Active Listening, And The Ability To Build And Maintain Relationships.

  • Experience In Healthcare: Previous Experience In The Healthcare Industry Is Essential For A Hospital Liaison. This May Include Working In A Hospital Setting, Home Health Care, Or A Similar Role That Involves Coordinating Patient Care And Managing Relationships With Healthcare Providers.

  • Proficiency In Medical Terminology: A Strong Understanding Of Medical Terminology Is Crucial For A Hospital Liaison. This Includes Knowledge Of Medical Procedures, Diagnoses, And Medications, Which Will Help In Accurately Communicating With Hospital Staff And Patients.

  • Organizational And Time Management Skills: A Successful Hospital Liaison Must Be Highly Organized And Able To Manage Their Time Efficiently. This Role Requires Juggling Multiple Tasks And Responsibilities, Such As Coordinating Patient Transfers And Managing Insurance Authorizations, So Strong Organizational Skills Are A Must.
