  1. Provide emotional support and counseling to patients and their families during difficult and vulnerable times.
  2. Conduct thorough assessments of patients' social and emotional needs and develop individualized care plans.
  3. Collaborate with healthcare team members to ensure integrated and comprehensive care for patients.
  4. Assist patients and families in navigating the healthcare system and connecting them with appropriate resources and support services.
  5. Advocate for patients' rights and needs, while maintaining ethical and professional standards.
  6. Maintain accurate and timely documentation of patient interactions and progress.
  7. Participate in multidisciplinary meetings and provide input on patients' social and emotional needs.
  8. Stay up-to-date on current social work practices, policies, and regulations.
  9. Communicate effectively with patients, families, and healthcare team members to promote understanding and cooperation.
  10. Demonstrate empathy, compassion, and cultural sensitivity in all interactions with patients and families.
  11. Collaborate with community organizations and agencies to develop and maintain relationships for referral purposes.
  12. Adhere to all HIPAA and confidentiality guidelines.
  13. Attend trainings and workshops to enhance knowledge and skills.
  14. Contribute to a positive and supportive work environment.
  15. Maintain a flexible schedule to meet the needs of patients and the organization.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • Current State Licensure As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw).

  • Minimum Of 2 Years Of Experience In A Clinical Social Work Role, Preferably In A Healthcare Setting.

  • Strong Knowledge Of Social Work Theories And Evidence-Based Interventions For Various Populations And Mental Health Conditions.

  • Ability To Work Independently And Collaboratively With A Multidisciplinary Team, Strong Communication And Organizational Skills, And A Commitment To Ethical And Culturally Sensitive Practice.
