LifePoint Health is seeking a full-time Social Work (SW) Case Manager to join our team and provide support and advocacy for our patients. In this role, you will have the opportunity to use your expertise in social work to assist patients and their families with accessing resources, coordinating care, and developing discharge plans. We are looking for a compassionate, dedicated individual with a strong understanding of social work principles and experience working with diverse populations.   1. Conduct psychosocial assessments of patients to identify their needs and develop appropriate care plans.
  2. Collaborate with healthcare team members to address the social, emotional, and psychological needs of patients.
  3. Utilize knowledge of community resources to assist patients and their families in accessing necessary support and services.
  4. Provide counseling and support to patients and their families to address emotional and behavioral challenges.
  5. Develop and implement discharge plans for patients, ensuring continuity of care and appropriate follow-up services.
  6. Advocate for patients and their families to ensure their rights and needs are met.
  7. Document all patient interactions and interventions accurately and in a timely manner.
  8. Attend and participate in interdisciplinary team meetings to discuss patient progress and develop comprehensive care plans.
  9. Stay up-to-date on social work practices and regulations to ensure compliance with industry standards.
  10. Maintain confidentiality of patient information and adhere to ethical standards of practice.
  11. Collaborate with insurance companies and other agencies to coordinate coverage and services for patients.
  12. Provide crisis intervention and support to patients and families in difficult situations.
  13. Participate in quality improvement initiatives and provide input on ways to improve the social work program.
  14. Communicate effectively with patients, families, and healthcare team members to ensure effective care coordination.
  15. Serve as a resource for colleagues and assist in training and mentoring new social work staff.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution

  • Minimum Of 2 Years Of Experience In Social Work Or Case Management, Preferably In A Healthcare Setting

  • Knowledge Of Local Community Resources And Social Service Programs

  • Strong Communication And Counseling Skills

  • Ability To Effectively Manage A Caseload And Meet Deadlines While Maintaining A High Level Of Organization And Attention To Detail
