- Coordinate and manage patient care in the Emergency Department, ensuring the highest level of quality and efficiency.
- Serve as a liaison between patients, families, and healthcare team members to ensure effective communication and collaboration.
- Develop and implement individualized care plans for patients, in collaboration with other healthcare professionals.
- Conduct regular assessments of patients' conditions and needs, and make appropriate changes to care plans as necessary.
- Advocate for patients and their families to ensure their needs and concerns are addressed.
- Utilize critical thinking skills to identify and address any potential barriers to patient care or discharge.
- Collaborate with insurance companies and other third-party payers to ensure appropriate coverage and reimbursement for patient care.
- Educate patients and their families on their healthcare needs, treatment plans, and available resources.
- Maintain accurate and thorough documentation of patient care in accordance with organizational and legal standards.
- Participate in quality improvement initiatives and contribute to the overall success of the emergency department.
- Stay up-to-date on industry trends and best practices in case management to continuously improve patient care and outcomes.
- Adhere to all organizational policies and procedures, as well as state and federal regulations related to patient care.
- Communicate effectively with other healthcare team members and actively participate in interdisciplinary rounds and meetings.
- Serve as a mentor and resource for other staff members, including new nurses and nursing students.
- Continuously evaluate and improve processes and procedures related to case management in the emergency department.
Valid Rn License: Candidates Must Possess An Active And Unrestricted Rn License In The State Where The Position Is Located. This Demonstrates That The Candidate Has Completed The Necessary Education And Training To Practice As A Registered Nurse.
Bsn Degree: A Bachelor Of Science In Nursing (Bsn) Is Preferred For This Position, As It Provides A More Comprehensive Education In Nursing Practice And Prepares Nurses For Leadership Roles. However, Candidates With An Associate Degree In Nursing (Adn) And Relevant Experience May Also Be Considered.
Minimum Of 2 Years Of Experience In Emergency Department (Ed) Nursing: The Ideal Candidate Will Have At Least 2 Years Of Experience Working In An Ed Setting. This Experience Allows Them To Be Familiar With The Fast-Paced And High-Stress Environment Of The Ed And To Have Developed The Necessary Skills And Knowledge To Effectively Manage Critical Patients.
Case Management Certification: Candidates Should Possess A Case Management Certification, Such As Certified Case Manager (Ccm) Or Accredited Case Manager (Acm). This Demonstrates A Strong Understanding Of Case Management Principles And Techniques, As Well As The Ability To Effectively Coordinate Patient Care And Navigate Complex Healthcare Systems.
Strong Communication And Interpersonal Skills: A Key Aspect Of The Role Of A Case Manager Ed Is To Communicate And Collaborate With A Variety Of Healthcare Professionals, Including Physicians, Nurses, And Social Workers. The Ideal Candidate Will Possess Strong Communication And Interpersonal Skills To Effectively Build Relationships And Work As Part Of A Multidisciplinary Team.
