  1. Provide high-quality social work services to patients by conducting assessments, developing treatment plans, and providing counseling and support.
  2. Collaborate with interdisciplinary team members to ensure the best possible outcomes for patients.
  3. Stay up-to-date on current social work practices, policies, and regulations to ensure compliance and provide the best care possible.
  4. Maintain accurate and thorough documentation of patient interactions and progress.
  5. Advocate for patients and their families to ensure they receive appropriate resources and support.
  6. Conduct outreach and education to raise awareness and promote access to services for underserved communities.
  7. Participate in team meetings and case conferences to discuss patient cases and provide input and recommendations.
  8. Develop and maintain relationships with community organizations and resources to support patient needs.
  9. Continuously assess and monitor patient progress and make adjustments to treatment plans as needed.
  10. Provide crisis intervention and support to patients in emergency situations.
  11. Conduct trainings and workshops for staff and community members on relevant social work topics.
  12. Adhere to ethical standards and maintain confidentiality in all patient interactions.
  13. Serve as a mentor and resource for social work interns and new team members.
  14. Collaborate with other departments within Kaiser Permanente to provide comprehensive care for patients.
  15. Participate in quality improvement initiatives to enhance the delivery of social work services.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University.

  • Minimum Of 5 Years Of Experience In A Healthcare Setting, Preferably In A Consulting Role.

  • Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Where Kaiser Permanente Operates.

  • Strong Knowledge Of Social Work Principles, Ethics, And Best Practices In A Healthcare Setting.

  • Excellent Communication, Collaboration, And Problem-Solving Skills, With The Ability To Work With Diverse Populations And Handle Sensitive Situations With Empathy And Professionalism.
