- Provide high-quality social work services to patients by conducting assessments, developing treatment plans, and providing counseling and support.
- Collaborate with interdisciplinary team members to ensure the best possible outcomes for patients.
- Stay up-to-date on current social work practices, policies, and regulations to ensure compliance and provide the best care possible.
- Maintain accurate and thorough documentation of patient interactions and progress.
- Advocate for patients and their families to ensure they receive appropriate resources and support.
- Conduct outreach and education to raise awareness and promote access to services for underserved communities.
- Participate in team meetings and case conferences to discuss patient cases and provide input and recommendations.
- Develop and maintain relationships with community organizations and resources to support patient needs.
- Continuously assess and monitor patient progress and make adjustments to treatment plans as needed.
- Provide crisis intervention and support to patients in emergency situations.
- Conduct trainings and workshops for staff and community members on relevant social work topics.
- Adhere to ethical standards and maintain confidentiality in all patient interactions.
- Serve as a mentor and resource for social work interns and new team members.
- Collaborate with other departments within Kaiser Permanente to provide comprehensive care for patients.
- Participate in quality improvement initiatives to enhance the delivery of social work services.
Master's Degree In Social Work From An Accredited University.
Minimum Of 5 Years Of Experience In A Healthcare Setting, Preferably In A Consulting Role.
Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Where Kaiser Permanente Operates.
Strong Knowledge Of Social Work Principles, Ethics, And Best Practices In A Healthcare Setting.
Excellent Communication, Collaboration, And Problem-Solving Skills, With The Ability To Work With Diverse Populations And Handle Sensitive Situations With Empathy And Professionalism.
