  3. Assess the learning needs of patients and provide individualized education plans to meet their specific needs.
  4. Utilize evidence-based practices and resources to enhance the effectiveness of health education programs.
  5. Serve as a resource for healthcare providers by providing up-to-date information on health education topics and trends.
  6. Conduct health screenings and assessments to identify potential health risks and provide appropriate education and interventions.
  7. Provide support and guidance to patients in understanding and navigating the healthcare system.
  8. Document patient education and interactions accurately and timely in electronic health records.
  9. Engage in continuous learning and professional development to stay current on best practices in health education.
  10. Act as an advocate for patients and their families, promoting health literacy and self-care management.
  11. Collaborate with community organizations to provide health education and resources to underserved populations.
  12. Participate in quality improvement initiatives to improve health outcomes and patient satisfaction.
  13. Adhere to all legal and ethical standards of nursing practice and maintain patient confidentiality.
  14. Communicate effectively with patients, families, and healthcare team members to ensure coordinated and comprehensive care.
  15. Maintain a positive and professional attitude and contribute to a collaborative and supportive work environment.

Job Qualifications
  • Current Registered Nurse (Rn) License In The State Of California

  • Bachelor's Degree In Nursing Or Related Field

  • Minimum Of 2 Years Of Experience In Health Education Or Related Field

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Educate And Engage Diverse Populations
