Population Care Manager

We’re looking for an experienced Population Care Manager to join Kaiser Permanente’s team. In this role, you’ll have the opportunity to make a real impact on the health and well-being of our members. Responsibilities:

1. Manage population health care plans for Kaiser Permanente members
2. Develop strategies to improve population health outcomes
3. Analyze data to identify trends in population health
4. Utilize meaningful use to develop and implement health care plans
5. Collaborate with other health care professionals to coordinate care for members
6. Monitor and evaluate population health care plans
7. Maintain up-to-date records of population health care plans
8. Provide support and guidance to members
9. Develop and implement educational programs to promote population health
10. Communicate with members and other health care professionals to ensure quality care
