Population Care Manager

We are looking for an experienced and compassionate Population Care Manager to join Kaiser Permanente’s Healthcare Team. In this role, you will be responsible for developing and executing processes to ensure quality care and positive outcomes for our members. We are looking for a leader who is passionate about healthcare, has a strong work ethic, and is dedicated to providing excellent service.To be successful in this role, you should have excellent organizational and communication skills, be detail-oriented and a team player. We are seeking an individual with a Bachelor’s degree in public health, healthcare administration, or related field, as well as at least 5 years of experience in healthcare management, population health care, or related field. A Master’s degree in public health, healthcare administration, or related field is preferred.
