Job Description

We are seeking an enthusiastic and hardworking Nutrition Assistant to join our team at Kaiser Permanente. In this role, you will support our health and nutrition team by helping to create nutrition care plans, conducting assessments, and providing educational resources to patients. To succeed in this role, you will need to have excellent communication and problem-solving skills, as well as a passion for helping others. A successful Nutrition Assistant will be organized, compassionate, and have a strong knowledge of nutrition and health principles. Responsibilities:

  1. Assist in creating nutrition care plans tailored to the individual needs of patients.
  2. Conduct assessments of patient health and nutrition status.
  3. Provide educational resources to patients to help them achieve their health and wellness goals.
  4. Maintain detailed records of patient interactions and progress.
  5. Collaborate with the health and nutrition team to ensure patient care is provided in a timely and efficient manner.
  6. Respond to patient inquiries and provide guidance in a professional and compassionate manner.
  7. Monitor patient progress and provide feedback to the health and nutrition team.
  8. Remain up-to-date on the latest nutrition and health principles and practices.
  9. Assist in the development and implementation of nutrition programs.
  10. Participate in professional development activities.
