Are you an experienced Nurse Practitioner or Physician Assistant with a passion for providing compassionate, patient-centered care?   1. Provide comprehensive palliative care services to patients in collaboration with a multidisciplinary team.
  2. Conduct thorough assessments of patients' physical, emotional, and spiritual needs to develop individualized care plans.
  3. Manage and treat complex medical conditions, including chronic pain, to improve patients' quality of life.
  4. Educate patients and families on palliative care options and support them in making informed decisions about their care.
  5. Serve as a resource and advocate for patients and families, ensuring their needs are met and their voices are heard.
  6. Maintain accurate and up-to-date medical records and documentation in accordance with organizational policies and procedures.
  7. Participate in team meetings and case conferences to discuss patient care plans and coordinate services with other healthcare professionals.
  8. Collaborate with primary care physicians and specialists to provide coordinated and integrated care for patients.
  9. Stay current on best practices and evidence-based guidelines in palliative care to continuously improve patient outcomes.
  10. Adhere to all ethical and legal standards in the delivery of care and maintain patient confidentiality at all times.
  11. Demonstrate a commitment to providing compassionate, patient-centered care and promoting a culture of empathy and understanding within the team.
  12. Participate in quality improvement initiatives and contribute to the development and implementation of policies and procedures related to palliative care.
  13. Comply with all organizational and regulatory requirements, including completing required trainings and maintaining licensure and certification.
  14. Contribute to a positive and collaborative work environment by fostering open communication and teamwork among team members.
  15. Represent the organization and the palliative care department in a professional manner at all times.

Job Qualifications
  • Master's Degree In Nursing Or Physician Assistant Studies From An Accredited Program.

  • Minimum Of 2 Years Of Experience As A Nurse Practitioner Or Physician Assistant In A Palliative Care Setting.

  • Current Licensure As A Nurse Practitioner Or Physician Assistant In The State Of Employment.

  • Board Certification In Palliative Care From A Recognized National Certification Board.

  • Demonstrated Ability To Work Collaboratively In A Multidisciplinary Team And Provide Compassionate Care To Patients With Complex Medical Conditions.
