  1. Conduct psychosocial assessments and develop individualized care plans for patients.
  2. Collaborate with interdisciplinary teams to address the social and emotional needs of patients.
  3. Provide counseling and support to patients and their families to help them cope with health-related challenges.
  4. Offer resources and referrals to community agencies and services to ensure continuity of care.
  5. Facilitate communication and coordination between patients, healthcare providers, and other involved parties.
  6. Document all interactions and interventions in a timely and accurate manner.
  7. Advocate for patients' rights and ensure their needs are met within the healthcare system.
  8. Monitor and address potential barriers to patients' discharge or transition to the next level of care.
  9. Participate in discharge planning and facilitate a smooth transition for patients from the hospital to their home or other care settings.
  10. Stay up-to-date with relevant laws, regulations, and best practices in medical social work.
  11. Maintain confidentiality and adhere to ethical standards in all patient interactions.
  12. Attend and actively contribute to team meetings and case conferences.
  13. Provide education and support to patients and their families on available resources and coping strategies.
  14. Identify and address social determinants of health that may impact patients' health outcomes.
  15. Act as a liaison between the hospital and community organizations to facilitate access to resources for patients.
  16. Continuously evaluate and improve the effectiveness of social work interventions for patients.
  17. Act as a resource and mentor for other healthcare professionals on social work principles and best practices.
  18. Demonstrate a commitment to diversity and cultural competency in all aspects of the job.
  19. Adhere to all hospital policies and procedures, including those related to confidentiality and patient rights.
  20. Participate in ongoing education and training to maintain and enhance professional skills and knowledge.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Program

  • Licensed Clinical Social Worker (Lcsw) In The State Of Employment

  • Minimum Of 2 Years Experience In A Medical Or Healthcare Setting, Preferably In An Inpatient Setting

  • Knowledge Of Medical Terminology, Hospital Systems, And Insurance Coverage

  • Ability To Assess Psychosocial Needs Of Patients And Provide Appropriate Interventions, Including Crisis Intervention And Discharge Planning.
