Job Description

We are looking for an experienced and compassionate Medical Social Worker to join our team at Kaiser Permanente. The Medical Social Worker will provide resources and support to individuals and families facing medical and social challenges and help them cope with the emotional and practical difficulties that arise from illness, disability, or other life-altering events. The ideal candidate must have a strong commitment to helping patients and their families make informed decisions as they navigate their care journey. We are looking for someone with excellent communication and interpersonal skills, a positive attitude, and a passion for providing high-quality care. Responsibilities:

  1. Develop and maintain relationships with patients and their families to provide emotional support and guidance.
  2. Provide resources, referrals, and support to help patients and their families make informed decisions about their care.
  3. Listen to patient and family concerns in order to identify and address social and emotional needs.
  4. Develop, implement, and evaluate individualized care plans for each patient and family.
  5. Advocate for patient rights and ensure they receive the best possible care.
  6. Collaborate with healthcare team members and other relevant stakeholders to ensure patient needs are met.
  7. Document patient care and progress in an electronic medical record.
  8. Maintain up-to-date knowledge of social services, medical resources, and community resources.
  9. Participate in continuing education and professional development activities.
  10. Comply with all applicable laws, regulations, and professional standards.
