Medical Social Worker

We are looking for an experienced and compassionate Medical Social Worker to join the Kaiser Permanente team. In this role, you will provide medical social services to patients and families, including case management and assistance in accessing community resources. The ideal candidate will possess exceptional interpersonal and communication skills, a commitment to promoting patient well-being, and a dedication to providing high quality, culturally sensitive services. Qualifications for this position include a Master’s degree in Social Work, a minimum of two years of clinical social work experience in a healthcare setting, and a valid license to practice social work in the state of practice. Responsibilities:

• Develop and implement individualized treatment plans for patients and families in need
• Provide case management services, including assessing and evaluating patient needs and developing interventions to address those needs
• Coordinate and collaborate with other healthcare professionals and community agencies to ensure that patients and families receive the best possible care
• Assist patients and families in accessing community resources, such as housing, food, and financial assistance
• Monitor patient progress and outcomes of interventions implemented
• Provide emotional support and guidance to patients and families
• Maintain patient confidentiality in accordance with HIPAA guidelines
• Maintain accurate and up-to-date documentation of patient care and progress in accordance with agency policies
• Participate in case conferences, team meetings, and other meetings as required
