  4. Connect patients and their families with community resources and support services as needed.
  5. Advocate for patients' rights and ensure they receive appropriate care.
  6. Facilitate support groups and educational programs for patients and their families.
  7. Maintain accurate and up-to-date records of patient interactions and progress.
  8. Continuously evaluate and monitor patients' progress and make necessary adjustments to treatment plans.

  10. Stay up-to-date on healthcare policies and regulations to ensure compliance with legal and ethical standards.
  11. Participate in interdisciplinary team meetings to discuss patient cases and develop holistic treatment plans.
  12. Educate patients and their families about coping strategies and self-care techniques.
  13. Collaborate with community organizations to develop and implement programs that address social and health-related issues.
  14. Act as a liaison between patients, their families, and healthcare providers to ensure effective communication and coordination of care.
  15. Uphold the values and mission of the organization by providing compassionate and patient-centered care.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Program

  • Valid State License To Practice Social Work

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In A Hospital Or Medical Clinic

  • Knowledge Of Medical Terminology, Healthcare Systems, And Resources Available For Patients

  • Strong Communication And Interpersonal Skills, Ability To Work Collaboratively With A Multidisciplinary Team
