  1. Conduct thorough assessments of patients' medical, emotional, and social needs.

  4. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive care for patients.
  5. Connect patients with appropriate community resources and services to meet their social and emotional needs.
  6. Advocate for patients' rights and advocate for necessary resources and support.
  7. Document patient progress and interventions accurately and timely.
  8. Stay current on medical and social issues and participate in ongoing education and training.
  9. Adhere to ethical and professional standards and maintain patient confidentiality.
  10. Collaborate with insurance companies and other third-party payers to ensure proper coverage for patients' needs.
  11. Participate in interdisciplinary team meetings and provide valuable insights and recommendations for patient care.
  12. Assist in discharge planning and ensure a smooth transition for patients from the hospital to their home or community.
  13. Serve as a resource and provide guidance to other healthcare team members on social and emotional issues related to patient care.
  14. Continuously evaluate and improve services and processes to meet the changing needs of patients.
  15. Maintain a positive and professional attitude and contribute to a supportive and collaborative work environment.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University

  • Current State License As A Licensed Clinical Social Worker (Lcsw)

  • Minimum Of 2 Years Of Experience Working As A Medical Social Worker In A Healthcare Setting

  • Knowledge And Understanding Of Medical Terminology, Healthcare Systems, And Insurance Coverage

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With A Diverse Patient Population And Collaborate With Healthcare Team Members.
