  1. Conduct comprehensive assessments of patients’ social, emotional, and psychological needs.
  2. Collaborate with a multidisciplinary team to develop individualized treatment plans for patients.
  3. Provide counseling and support to patients and their families during challenging medical situations.
  4. Utilize knowledge of community resources to assist patients in accessing necessary services.
  5. Advocate for patients and their families to ensure their needs are being met.
  6. Document patient interactions and progress in accordance with hospital and state regulations.
  7. Maintain up-to-date knowledge of social work practices and theories.
  8. Attend and participate in team meetings and case conferences.
  9. Provide crisis intervention and support to patients experiencing mental health or social issues.
  10. Communicate effectively with patients, families, and other healthcare professionals.
  11. Collaborate with insurance companies and other agencies to coordinate care for patients.
  12. Adhere to ethical standards and maintain patient confidentiality.
  13. Continuously evaluate and update treatment plans to meet the changing needs of patients.
  14. Participate in ongoing professional development and training programs.
  15. Demonstrate empathy, compassion, and cultural sensitivity towards patients from diverse backgrounds.
  16. Maintain a positive and professional attitude towards patients and colleagues.
  17. Adhere to all hospital policies and procedures.
  18. Represent Kaiser Permanente positively in the community.

Job Qualifications
  • A Master's Degree In Social Work From An Accredited Institution.

  • A Valid License To Practice Social Work In The State Where The Position Is Located.

  • Minimum Of 2 Years Of Experience Working In A Healthcare Setting, Preferably In A Hospital Or Clinic.

  • Strong Understanding Of Medical Terminology And Procedures, As Well As Knowledge Of Insurance And Government Programs.

  • Excellent Communication And Counseling Skills, With The Ability To Work With Diverse Patient Populations And Collaborate With Interdisciplinary Teams.
