Job Description

Welcome to Kaiser Permanente! We are currently seeking a compassionate and dedicated LPN to join our team as a Post-Discharge Patient Phone Contact. In this role, you will play a crucial role in ensuring that our patients receive the best possible care even after they leave our facilities. Your warm and empathetic nature will be essential in providing support and guidance to patients during their transition from hospital to home.   1. Conduct post-discharge phone calls to patients to ensure they are receiving proper care and support at home.
  2. Provide emotional support and guidance to patients during their transition from hospital to home.
  3. Collaborate with healthcare team to develop individualized care plans for patients.
  4. Monitor and document patient progress and relay any concerns to healthcare team.
  5. Educate patients and their families on proper self-care techniques and medication management.
  6. Coordinate and schedule follow-up appointments for patients as needed.
  7. Maintain accurate and up-to-date patient records.
  8. Adhere to all HIPAA regulations and maintain patient confidentiality.
  9. Continuously demonstrate empathy and compassion towards patients and their families.
  10. Participate in ongoing training and development to stay updated on best practices and advancements in patient care.
  11. Foster a positive and welcoming environment for patients and their families.
  12. Collaborate with other healthcare professionals to ensure seamless and coordinated patient care.
  13. Adhere to all hospital policies, procedures, and safety regulations.
  14. Demonstrate excellent communication skills and build rapport with patients and their families.
  15. Perform other duties as assigned by supervisor.

Job Qualifications
  • Valid Lpn License: Candidates Must Possess A Valid And Active Lpn License In The State Where They Will Be Working.

  • Previous Experience In A Post-Discharge Role: Candidates Should Have At Least 2 Years Of Experience Working In A Post-Discharge Or Similar Patient Contact Role In A Healthcare Setting.

  • Strong Communication Skills: The Lpn Must Be Able To Effectively Communicate With Patients Over The Phone, As Well As Collaborate With Other Healthcare Team Members.

  • Knowledge Of Patient Care And Assessment: The Candidate Should Have A Thorough Understanding Of Patient Care And Be Able To Assess And Address Any Post-Discharge Concerns Or Issues.

  • Familiarity With Electronic Medical Records (Emr): Knowledge Of Emrs And Ability To Navigate And Document Patient Interactions Is Essential For This Role. Experience With Kaiser Permanente's Specific Emr System Is Preferred.
