
LPN - Post-Discharge Patient Phone Contact

- Conduct post-discharge phone calls to patients to ensure they are receiving proper care and support at home.
- Provide emotional support and guidance to patients during their transition from hospital to home.
- Collaborate with healthcare team to develop individualized care plans for patients.
- Monitor and document patient progress and relay any concerns to healthcare team.
- Educate patients and their families on proper self-care techniques and medication management.
- Coordinate and schedule follow-up appointments for patients as needed.
- Maintain accurate and up-to-date patient records.
- Adhere to all HIPAA regulations and maintain patient confidentiality.
- Continuously demonstrate empathy and compassion towards patients and their families.
- Participate in ongoing training and development to stay updated on best practices and advancements in patient care.
- Foster a positive and welcoming environment for patients and their families.
- Collaborate with other healthcare professionals to ensure seamless and coordinated patient care.
- Adhere to all hospital policies, procedures, and safety regulations.
- Demonstrate excellent communication skills and build rapport with patients and their families.
- Perform other duties as assigned by supervisor.
Valid Lpn License: Candidates Must Possess A Valid And Active Lpn License In The State Where They Will Be Working.
Previous Experience In A Post-Discharge Role: Candidates Should Have At Least 2 Years Of Experience Working In A Post-Discharge Or Similar Patient Contact Role In A Healthcare Setting.
Strong Communication Skills: The Lpn Must Be Able To Effectively Communicate With Patients Over The Phone, As Well As Collaborate With Other Healthcare Team Members.
Knowledge Of Patient Care And Assessment: The Candidate Should Have A Thorough Understanding Of Patient Care And Be Able To Assess And Address Any Post-Discharge Concerns Or Issues.
Familiarity With Electronic Medical Records (Emr): Knowledge Of Emrs And Ability To Navigate And Document Patient Interactions Is Essential For This Role. Experience With Kaiser Permanente's Specific Emr System Is Preferred.
