We are currently seeking a Licensed Graduate Social Worker (LGSW) to join our team as a Case Manager. As a Case Manager, you will play a critical role in advocating for and supporting our members' social and emotional well-being.   1. Provide compassionate and professional support to Kaiser Permanente members as a Licensed Graduate Social Worker (LGSW).
  2. Advocate for the social and emotional well-being of members by assessing their needs and developing individualized care plans.
  3. Collaborate with healthcare providers and interdisciplinary teams to ensure a holistic approach to member care.
  4. Monitor and evaluate member progress, making necessary adjustments to care plans as needed.
  5. Conduct regular assessments to identify potential barriers to care and develop strategies to address them.
  6. Serve as a liaison between members, healthcare providers, and community resources to connect members with necessary services.
  7. Maintain accurate and timely documentation of member interactions and progress in electronic medical records.
  8. Stay up-to-date on best practices and resources in the field of social work to provide high-quality care to members.
  9. Comply with all ethical and legal standards in social work practice.
  10. Demonstrate a strong commitment to diversity, equity, and inclusion in all interactions with members and colleagues.
  11. Participate in ongoing training and professional development opportunities to enhance skills and knowledge.
  12. Serve as a positive representative of Kaiser Permanente and uphold the organization's values and mission.
  13. Adhere to all relevant regulations and guidelines, including HIPAA and state licensing requirements.
  14. Participate in team meetings and collaborate with colleagues to ensure a cohesive and effective approach to member care.
  15. Continuously strive to improve processes and procedures to enhance the member experience and outcomes.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • Current Licensure As A Licensed Graduate Social Worker (Lgsw) In The State Of Employment.

  • Minimum Of 2 Years Of Experience In Case Management Or Related Social Work Field.

  • Demonstrated Knowledge Of Community Resources And Ability To Collaborate With Interdisciplinary Teams.

  • Excellent Communication, Organizational, And Problem-Solving Skills.
