
Inpatient Psychiatric Case Manager

- Collaborate with healthcare providers, insurance companies, and other relevant parties to ensure the best possible care for patients.
- Utilize knowledge and experience in mental health to assess patients' needs and develop individualized care plans.
- Serve as a liaison between patients and their families, providing updates on treatment plans and addressing any concerns or questions.
- Monitor patients' progress and make necessary adjustments to treatment plans as needed.
- Conduct regular assessments and evaluations to ensure the effectiveness of treatment and make recommendations for improvement.
- Educate patients and their families on treatment options, resources, and support services available.
- Maintain accurate and up-to-date documentation and records of patient care and progress.
- Collaborate with interdisciplinary team members to ensure a comprehensive and holistic approach to patient care.
- Stay current on industry trends, best practices, and changes in healthcare policies and regulations.
- Demonstrate compassion, empathy, and sensitivity in all interactions with patients and their families.
- Adhere to all ethical and legal standards, maintaining confidentiality and privacy of patient information.
- Participate in team meetings, trainings, and other professional development opportunities to enhance skills and knowledge.
- Represent Kaiser Permanente in a professional and positive manner, promoting the organization's mission and values.

Bachelor's Or Master's Degree In Social Work, Psychology, Or Related Field.
Minimum Of 2-3 Years Of Experience Working In A Psychiatric Or Mental Health Setting.
Knowledge Of Evidence-Based Treatment Modalities For Mental Health And Substance Use Disorders.
Experience With Case Management, Including Assessment, Treatment Planning, And Coordination Of Care.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With A Multi-Disciplinary Team And Work With Diverse Patient Populations.
