
Inpatient Psychiatric Case Manager

- Coordinate and manage the care of inpatient psychiatric patients at Kaiser Permanente.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans for patients.
- Conduct assessments and evaluations to determine the most appropriate course of treatment for patients.
- Monitor and track patient progress, identifying any changes or concerns and making appropriate adjustments to treatment plans.
- Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and understanding of treatment plans.
- Provide education and support to patients and families regarding mental health conditions, treatment options, and community resources.
- Maintain accurate and timely documentation of patient care and treatment plans.
- Collaborate with insurance companies and other agencies to ensure appropriate coverage for patients' treatment.
- Stay up-to-date on current practices, research, and trends in psychiatric care to provide the best possible care to patients.
- Adhere to all legal and ethical standards in patient care and maintain patient confidentiality.
- Participate in multidisciplinary team meetings and contribute to the development and improvement of psychiatric services at Kaiser Permanente.
- Demonstrate compassion, empathy, and sensitivity towards patients and their families.
- Uphold the mission, vision, and values of Kaiser Permanente in all interactions and responsibilities.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Minimum Of 2 Years Of Experience Working In A Mental Health Setting, Preferably In A Case Management Role.
Knowledge And Understanding Of Mental Health Diagnoses, Treatment Options, And Community Resources.
Strong Communication And Interpersonal Skills To Effectively Collaborate With Multidisciplinary Teams And Advocate For Patients' Needs.
Experience With Electronic Medical Records And Ability To Document Patient Progress And Treatment Plans Accurately.
