- Review and verify medication orders to ensure accuracy and appropriateness for patients' conditions and treatment plans.
- Collaborate with physicians and other healthcare professionals to develop and adjust medication therapy plans.
- Monitor patients' response to medications and make appropriate interventions as needed.
- Provide drug information and education to patients, caregivers, and healthcare staff.
- Maintain accurate and complete patient medication records and documentation.
- Ensure compliance with all state and federal laws and regulations related to medication use.
- Participate in multidisciplinary rounds and provide recommendations for medication therapy.
- Coordinate with pharmacy technicians to ensure timely and accurate medication preparation and dispensing.
- Conduct medication reconciliation to prevent errors and improve patient safety.
- Stay current with new medications, drug therapies, and pharmacy practice guidelines.
- Contribute to quality improvement initiatives and participate in medication-related committees.
- Act as a resource for other healthcare professionals regarding medication therapy and drug information.
- Provide exceptional customer service and patient care at all times.
- Maintain confidentiality and uphold ethical standards in all aspects of job performance.
- Serve as a preceptor for pharmacy students and residents.

Active Pharmacist License In The State Of Employment
Doctor Of Pharmacy (Pharmd) Degree From An Accredited University
Completion Of A Post-Graduate Residency Program In Inpatient Pharmacy
Board Certification In Pharmacotherapy (Bcps) Or A Related Specialty
Minimum Of 2 Years Of Experience As A Clinical Pharmacist In An Inpatient Hospital Setting
