  1. Review and verify medication orders to ensure accuracy and appropriateness for patients' conditions and treatment plans.
  2. Collaborate with physicians and other healthcare professionals to develop and adjust medication therapy plans.
  3. Monitor patients' response to medications and make appropriate interventions as needed.
  4. Provide drug information and education to patients, caregivers, and healthcare staff.
  5. Maintain accurate and complete patient medication records and documentation.
  6. Ensure compliance with all state and federal laws and regulations related to medication use.
  7. Participate in multidisciplinary rounds and provide recommendations for medication therapy.
  8. Coordinate with pharmacy technicians to ensure timely and accurate medication preparation and dispensing.
  9. Conduct medication reconciliation to prevent errors and improve patient safety.
  10. Stay current with new medications, drug therapies, and pharmacy practice guidelines.
  11. Contribute to quality improvement initiatives and participate in medication-related committees.
  12. Act as a resource for other healthcare professionals regarding medication therapy and drug information.
  13. Provide exceptional customer service and patient care at all times.
  14. Maintain confidentiality and uphold ethical standards in all aspects of job performance.
  15. Serve as a preceptor for pharmacy students and residents.
  16. Participate in continuing education and professional development activities.
  17. Adhere to departmental policies and procedures.
  18. Perform any other duties as assigned by the pharmacy manager or director.

Job Qualifications
  • Active Pharmacist License In The State Of Employment

  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited University

  • Completion Of A Post-Graduate Residency Program In Inpatient Pharmacy

  • Board Certification In Pharmacotherapy (Bcps) Or A Related Specialty

  • Minimum Of 2 Years Of Experience As A Clinical Pharmacist In An Inpatient Hospital Setting
