  1. Dispense and monitor medications for hospitalized patients according to established protocols and procedures.
  2. Collaborate with healthcare team members to develop and implement individualized medication treatment plans.
  3. Conduct thorough medication reviews to ensure accuracy, appropriateness, and effectiveness of prescribed medications.
  4. Provide drug information and education to patients, caregivers, and healthcare professionals.
  5. Utilize electronic health records and other technological systems to document and track medication therapy.
  6. Identify and resolve medication-related problems and adverse drug reactions.
  7. Monitor and manage medication inventory to ensure availability and proper storage of medications.
  8. Comply with all federal, state, and local laws and regulations related to pharmacy practice.
  9. Maintain accurate and up-to-date patient records and medication profiles.
  10. Participate in quality improvement initiatives and medication safety protocols.
  11. Stay current with new developments and trends in pharmacy practice and healthcare.
  12. Maintain professional relationships with other healthcare providers and participate in interdisciplinary rounds.
  13. Provide excellent customer service to patients and their families.
  14. Train and supervise pharmacy technicians and interns.
  15. Adhere to ethical and professional standards of conduct at all times.

Job Qualifications
  • Active Pharmacist License In The State Of Employment

  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution

  • Minimum Of 2 Years Of Experience In An Inpatient Pharmacy Setting

  • Knowledge Of Hospital Pharmacy Operations, Including Medication Order Entry, Dispensing, And Clinical Monitoring

  • Strong Communication And Interpersonal Skills, With Ability To Work Collaboratively With A Multidisciplinary Team Of Healthcare Professionals.
