- Dispense medications accurately and efficiently to hospitalized patients in accordance with hospital and state regulations.
- Monitor and review patient medication orders for appropriateness, drug interactions, and adverse reactions.
- Collaborate with healthcare team members to develop and implement treatment plans for patients.
- Provide drug information and education to patients, caregivers, and healthcare professionals.
- Maintain accurate and complete records of medication dispensing and patient information.
- Participate in medication reconciliation processes to ensure safe transitions of care for patients.
- Perform medication therapy management and make recommendations for optimizing medication regimens.
- Ensure compliance with medication storage, handling, and disposal protocols.
- Stay up-to-date on the latest developments and advancements in pharmaceuticals and healthcare.
- Adhere to all safety, health, and infection control policies and procedures.
- Train and supervise pharmacy technicians and interns.
- Participate in multidisciplinary team meetings and contribute to quality improvement initiatives.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Provide excellent customer service to patients, families, and healthcare team members.
- Collaborate with pharmacy purchasing and inventory management to ensure adequate medication supply.
- Follow all departmental and organizational policies and procedures.
- Participate in continuing education and professional development activities.
- Serve as a resource and mentor to other healthcare professionals regarding medication-related issues.
- Maintain a professional and respectful attitude towards patients, colleagues, and the organization.
- Demonstrate a commitment to the mission, vision, and values of Kaiser Permanente.
A Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School.
A Valid State License To Practice Pharmacy.
At Least 2 Years Of Experience Working In An Inpatient Pharmacy Setting.
Knowledge And Understanding Of Hospital Pharmacy Operations, Including Medication Dispensing, Inventory Management, And Medication Reconciliation.
Excellent Communication And Interpersonal Skills To Effectively Collaborate With Healthcare Professionals, Patients, And Their Families.
