
Inpatient Pharmacist

- Dispense medications to inpatients accurately and in a timely manner following all applicable laws, regulations, and standards of practice.
- Ensure proper storage and handling of all medications, including controlled substances.
- Review medication orders for appropriateness, checking for drug interactions, allergies, and contraindications.
- Collaborate with physicians and other healthcare professionals to select the most appropriate medications for each patient.
- Provide medication counseling and education to patients and their families.
- Maintain accurate and complete patient medication records.
- Monitor patient response to medications and make necessary adjustments as needed.
- Participate in medication reconciliation processes to ensure continuity of care.
- Conduct drug utilization reviews and provide recommendations for improving medication use and reducing costs.
- Work closely with the pharmacy team to ensure adequate medication supply and assist with inventory management.
- Stay up-to-date on new medications, therapies, and treatment guidelines.
- Regularly assess and improve medication management processes to ensure patient safety and quality of care.
- Adhere to all Kaiser Permanente policies, procedures, and protocols.
- Maintain confidentiality and adhere to all HIPAA regulations.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning and improvement.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Or University.
Current And Active Pharmacist License In The State Of Employment.
At Least 2 Years Of Experience As A Pharmacist In An Inpatient Setting.
Knowledge Of Inpatient Pharmacy Operations, Including Medication Dispensing, Iv Compounding, And Clinical Pharmacy Services.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Healthcare Providers And Patients To Ensure Safe And Effective Medication Use.
