- Dispensing and verifying accurate medications for inpatient use.
- Collaborating with physicians and other healthcare professionals to ensure appropriate and effective medication therapy.
- Monitoring and evaluating patient response to medications and making necessary adjustments.
- Providing drug information and education to patients, caregivers, and healthcare team members.
- Maintaining accurate and complete patient medication records.
- Participating in patient care rounds and providing clinical recommendations.
- Adhering to all state and federal regulations related to medication use and storage.
- Ensuring compliance with hospital policies and procedures for medication management.
- Communicating with insurance companies to obtain prior authorizations for medications.
- Participating in medication safety initiatives and quality improvement projects.
- Mentoring and training pharmacy students and residents.
- Maintaining current knowledge of medications, disease states, and new developments in pharmacy practices.
- Collaborating with pharmacy technicians to ensure accurate and timely medication preparation.
- Identifying and reporting adverse drug reactions and medication errors.
- Participating in on-call and weekend coverage as needed.
Pharmd Degree From An Accredited Pharmacy School.
Active And Unrestricted Pharmacist License In The State Of Employment.
Minimum Of 1-2 Years Of Experience As An Inpatient Pharmacist, Preferably In A Hospital Or Healthcare Setting.
Strong Knowledge Of Medication Therapy Management, Drug Interactions, And Pharmacotherapy.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Healthcare Teams And Patients.
