Responsibilities:

1. Provide compassionate and skilled home health social work services to Kaiser Permanente members.
2. Assess, plan, and implement individualized care plans to meet members' unique needs.
3. Maintain accurate and up-to-date records of members' progress.
4. Develop and maintain positive relationships with members and their families.
5. Advocate for members' rights and best interests.
6. Remain current on relevant laws, regulations, and best practices related to home health social work.
7. Attend and participate in regular team meetings and workshops.
8. Provide guidance and support to members in times of crisis.
9. Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
10. Collaborate with other health care professionals to coordinate member care.
