Home Health Assistant

Responsibilities:

1. Assist members with daily activities that promote their safety and well-being
2. Provide compassionate and attentive care to members in their homes
3. Respond to member needs in a timely and effective manner
4. Develop and maintain accurate records of home health services provided
5. Assist members in setting and achieving health goals
6. Work collaboratively with other medical staff to meet members’ needs
7. Ensure compliance with company policies and regulations
8. Maintain reliable transportation to travel to members’ homes
9. Administer medications and treatments as prescribed by medical staff
10. Provide education and support to members and their families
