  1. Coordinate patient care by working closely with healthcare providers, insurance companies, and patients to ensure timely and appropriate treatment plans.
  2. Utilize case management principles to assess patient needs and develop individualized care plans.
  3. Collaborate with interdisciplinary team members to ensure continuity of care and appropriate utilization of resources.
  4. Communicate effectively with patients, families, and caregivers to provide education and support regarding treatment plans and available resources.
  5. Monitor and evaluate patient progress, identifying any potential barriers to care and implementing solutions.
  6. Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and any necessary authorizations.
  7. Advocate for patients' needs and advocate for necessary services or resources.
  8. Participate in quality improvement initiatives to enhance patient care and outcomes.
  9. Stay updated on current healthcare regulations and policies to ensure compliance.
  10. Serve as a resource for patients, families, and healthcare providers regarding case management processes and services.
  11. Provide support and guidance to other team members, including case managers and support staff.
  12. Participate in case conferences and meetings to discuss patient care and make recommendations for improvement.
  13. Continuously seek opportunities to improve patient care and streamline processes.
  14. Maintain confidentiality and adhere to all HIPAA guidelines when handling patient information.
  15. Uphold the mission, vision, and values of the organization in all aspects of the role.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or Related Field.

  • Minimum Of 3 Years Experience In Case Management Or Care Coordination.

  • Working Knowledge Of Healthcare Systems, Insurance Plans, And Regulatory Requirements.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Interdisciplinary Teams.

  • Prior Experience With Electronic Health Records And Care Management Software.
