- Participate in case conferences and meetings to discuss patient care and make recommendations for improvement.

- Maintain confidentiality and adhere to all HIPAA guidelines when handling patient information.

Bachelor's Degree In Nursing, Social Work, Or Related Field.
Minimum Of 3 Years Experience In Case Management Or Care Coordination.
Working Knowledge Of Healthcare Systems, Insurance Plans, And Regulatory Requirements.
Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Interdisciplinary Teams.
Prior Experience With Electronic Health Records And Care Management Software.
