  1. Oversee and manage the day-to-day operations of the Home Health Hospice team.
  2. Provide leadership and guidance to a team of clinicians, including nurses, therapists, and support staff.
  3. Ensure that all patient care services are delivered in accordance with established standards and protocols.
  4. Monitor and evaluate the performance of the team, providing feedback and coaching as needed.
  5. Collaborate with other healthcare professionals, including physicians and case managers, to coordinate care and ensure optimal patient outcomes.
  6. Develop and implement policies and procedures to enhance the efficiency and effectiveness of the team.
  7. Ensure compliance with all regulatory and accreditation requirements.
  8. Manage staffing and scheduling to ensure adequate coverage and timely delivery of services.
  9. Participate in hiring, training, and onboarding of new team members.
  10. Foster a positive and collaborative work environment that promotes teamwork and continuous learning.
  11. Stay updated on developments and advancements in the field of home health and hospice care.
  12. Serve as a liaison between the team and other departments within the organization.
  13. Address and resolve any issues or concerns raised by patients or their families.
  14. Participate in quality improvement initiatives to enhance the delivery of care.
  15. Act as a role model for the team by demonstrating professionalism, empathy, and compassion in all interactions.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Healthcare Field.

  • Minimum Of 3-5 Years Of Experience In A Supervisory Or Management Role In A Home Health Or Hospice Setting.

  • Current State Nursing License.

  • Knowledge Of Medicare And Medicaid Regulations Related To Home Health And Hospice Services.

  • Excellent Communication And Interpersonal Skills, With The Ability To Lead And Motivate A Team Of Healthcare Professionals.
