
Clinical Services Supervisor - Home Health Hospice

- Oversee and manage a team of healthcare professionals in providing high-quality care to patients in their homes.
- Ensure that all team members adhere to the organization's policies, procedures, and standards of care.
- Monitor and assess the team's performance, providing feedback and coaching as needed to maintain a high level of quality and productivity.
- Collaborate with other department leaders to develop and implement strategies to improve patient outcomes and increase efficiency.
- Maintain accurate and up-to-date records of team members' credentials, training, and performance evaluations.
- Foster a positive and supportive work environment for team members, promoting teamwork, open communication, and professional development.
- Ensure compliance with all regulatory and accreditation requirements related to home health and hospice care.
- Act as a liaison between the team and other healthcare providers, patients, and their families to facilitate coordinated and effective care.
- Utilize data and metrics to track and improve team performance and identify areas for improvement.
- Participate in hiring, onboarding, and training of new team members.
- Stay current with industry developments and best practices in home health and hospice care and incorporate them into the team's practices.
- Handle any conflicts or issues that arise within the team, providing timely and effective resolution.
- Serve as a role model for compassionate and patient-centered care, leading by example and promoting a culture of empathy and respect.
- Represent the organization in a professional and positive manner at all times.
- Adhere to all confidentiality and HIPAA regulations in handling patient information.
- Collaborate with other departments to develop and implement policies and procedures to ensure the safe and efficient delivery of care.
- Communicate regularly and effectively with upper management to report on the team's performance and provide updates on any significant issues or developments.
- Continuously evaluate and improve processes and workflows to enhance the team's effectiveness and efficiency.
- Conduct regular team meetings and provide ongoing training and education
Minimum Of 5 Years Experience In Home Health And Hospice Services, With At Least 2 Years In A Supervisory Or Leadership Role.
Bachelor's Degree In Nursing Or Related Field, With A Current Rn License In The State Of Employment.
Strong Knowledge Of Federal And State Regulations Related To Home Health And Hospice Services, As Well As Experience With Accreditation And Survey Processes.
Excellent Communication And Interpersonal Skills, With The Ability To Effectively Lead And Mentor A Team Of Clinicians.
Demonstrated Experience In Developing And Implementing Quality Improvement Initiatives And Ensuring Compliance With Clinical Best Practices.
