  1. Oversee the daily operations of the Home Health Hospice department, ensuring that all services are delivered efficiently and with the highest level of quality.
  2. Manage and supervise a team of healthcare professionals, including nurses, therapists, and aides, to ensure they are providing exceptional care to patients.
  3. Develop and implement policies and procedures to improve the overall efficiency and effectiveness of the department.
  4. Collaborate with other departments and healthcare professionals to ensure seamless coordination of care for patients.
  5. Monitor and evaluate the performance of staff, providing regular feedback and coaching to maintain high standards of care.
  6. Conduct regular audits and assessments to ensure compliance with all regulatory guidelines and standards.
  7. Act as a liaison between the department and other departments within Kaiser Permanente, as well as external agencies and organizations.
  8. Participate in the recruitment, hiring, and training of new staff members.
  9. Develop and maintain relationships with patients and their families, addressing any concerns or issues that may arise.
  10. Stay updated on industry trends and best practices in home health and hospice care, implementing new strategies and techniques as needed.
  11. Foster a positive and supportive work environment for all team members, promoting a culture of empathy, compassion, and teamwork.
  12. Conduct regular meetings with staff to ensure open communication, address any challenges, and celebrate successes.
  13. Maintain accurate and thorough records of patient care and department activities.
  14. Handle any escalated issues or emergencies in a timely and effective manner.
  15. Continuously seek opportunities to improve and enhance the services provided by the department.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing Or Related Field

  • Minimum Of 3-5 Years Of Experience In Home Health Or Hospice Care

  • Previous Supervisory Experience In A Clinical Setting

  • Knowledge Of Medicare And Medicaid Regulations And Reimbursement Processes

  • Strong Leadership Skills And Ability To Effectively Manage And Motivate A Team Of Healthcare Professionals.
