  1. Provide support and guidance to clinical staff in the area of case management.
  2. Ensure compliance with industry standards and best practices in case management.
  3. Collaborate with other healthcare professionals to develop and implement effective case management strategies.
  4. Conduct regular assessments of patient progress and adjust case management plans as needed.
  5. Develop and maintain relationships with external partners and resources to enhance patient care.
  6. Keep up-to-date on the latest developments and trends in case management and share knowledge with clinical staff.
  7. Develop and implement training programs for clinical staff on case management best practices.
  8. Monitor and evaluate the effectiveness of case management protocols and make recommendations for improvements.
  9. Ensure timely and accurate documentation of case management activities.
  10. Act as a resource and advocate for patients and their families, addressing any concerns or issues related to case management.
  11. Collaborate with other departments within the organization to promote a coordinated and comprehensive approach to patient care.
  12. Participate in quality assurance initiatives and assist with developing and implementing quality improvement plans.
  13. Demonstrate a commitment to continuous learning and professional development in the field of case management.
  14. Adhere to all confidentiality and privacy policies and regulations.
  15. Represent Kaiser Permanente and the case management team in a professional and positive manner.

Job Qualifications
  • Current And Active Rn License In The State Of Employment.

  • Bachelor's Degree In Nursing Or Related Healthcare Field, With A Preference For A Master's Degree In Nursing, Healthcare Administration, Or A Related Field.

  • Minimum Of 5 Years Of Experience In Case Management, Preferably In A Hospital Or Healthcare Setting.

  • Strong Knowledge Of Healthcare Regulations And Standards, Including Medicare And Medicaid Guidelines.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With A Multidisciplinary Team And Effectively Communicate With Patients, Families, And Other Healthcare Providers.
