
Clinical Practice Consultant - Case Management

- Provide support and guidance to clinical staff in the area of case management.

- Collaborate with other healthcare professionals to develop and implement effective case management strategies.
- Conduct regular assessments of patient progress and adjust case management plans as needed.
- Develop and maintain relationships with external partners and resources to enhance patient care.
- Keep up-to-date on the latest developments and trends in case management and share knowledge with clinical staff.
- Develop and implement training programs for clinical staff on case management best practices.
- Monitor and evaluate the effectiveness of case management protocols and make recommendations for improvements.
- Ensure timely and accurate documentation of case management activities.
- Act as a resource and advocate for patients and their families, addressing any concerns or issues related to case management.
- Collaborate with other departments within the organization to promote a coordinated and comprehensive approach to patient care.
- Participate in quality assurance initiatives and assist with developing and implementing quality improvement plans.
- Demonstrate a commitment to continuous learning and professional development in the field of case management.
- Adhere to all confidentiality and privacy policies and regulations.
- Represent Kaiser Permanente and the case management team in a professional and positive manner.
Current And Active Rn License In The State Of Employment.
Bachelor's Degree In Nursing Or Related Healthcare Field, With A Preference For A Master's Degree In Nursing, Healthcare Administration, Or A Related Field.
Minimum Of 5 Years Of Experience In Case Management, Preferably In A Hospital Or Healthcare Setting.
Strong Knowledge Of Healthcare Regulations And Standards, Including Medicare And Medicaid Guidelines.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With A Multidisciplinary Team And Effectively Communicate With Patients, Families, And Other Healthcare Providers.
