- Collaborate with healthcare teams to develop and implement evidence-based treatment plans for patients with complex medication needs.

- Act as a liaison between the pharmacy department and other healthcare teams to promote collaboration and communication.
- Maintain accurate and thorough documentation of all patient interactions and interventions.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy
Completion Of A Post-Graduate Pharmacy Residency Program In Primary Care Or Ambulatory Care
Board Certification In Pharmacotherapy Or Ambulatory Care Pharmacy From A Recognized Organization (E.g. Board Of Pharmacy Specialties)
Experience Working As A Clinical Pharmacy Specialist In A Primary Care Setting
