- Coordinate and manage care for patients who require extended care services.
- Develop and implement individualized care plans based on patient needs and goals.
- Collaborate with healthcare team members to ensure coordinated and comprehensive care.
- Communicate with patients and their families to provide education and support regarding extended care services.
- Conduct assessments and evaluations to determine patient eligibility for extended care services.
- Monitor patient progress and adjust care plans as needed.
- Advocate for patients and ensure their needs and preferences are met.
- Maintain accurate and thorough documentation of patient care and progress.
- Work closely with insurance companies and other providers to facilitate coverage and payment for extended care services.
- Stay updated on industry trends and best practices in case management and extended care services.
- Participate in interdisciplinary team meetings and contribute to the development of care protocols and procedures.
- Adhere to all HIPAA and ethical standards in patient care and confidentiality.
- Provide emotional support and counseling to patients and their families during difficult times.
- Collaborate with community resources to ensure a smooth transition for patients from extended care services to their home or other care settings.
- Continuously evaluate and improve the quality of care provided to patients.
- Maintain a positive and professional attitude, representing the values and mission of Kaiser Permanente at all times.
Rn License: A Valid And Active Rn License Is Required For This Role, Demonstrating The Candidate's Knowledge And Competency In The Nursing Field.
Case Management Experience: Prior Experience In Case Management, Particularly In Extended Care Coordination, Is Preferred. This Includes Knowledge Of Discharge Planning, Utilization Management, And Care Coordination Processes.
Strong Communication Skills: Excellent Communication Skills, Both Verbal And Written, Are Essential For This Role. The Candidate Must Be Able To Effectively Communicate With Patients, Families, And Other Healthcare Professionals To Ensure Seamless Care Coordination.
Knowledge Of Medicare And Medicaid Regulations: A Thorough Understanding Of Medicare And Medicaid Regulations Is Important For This Position, As The Candidate Will Be Responsible For Ensuring Compliance And Identifying Any Potential Issues.
Ability To Work Independently: The Ideal Candidate Should Be Able To Work Independently, Managing A Caseload Of Patients And Coordinating Their Care From Start To Finish. This Requires Strong Organizational And Time-Management Skills, As Well As The Ability To Prioritize Tasks Effectively.
