Job Description

We are seeking an experienced Case Management RN to join our OBGYN team at Kaiser Permanente. The ideal candidate will be a compassionate, detail-oriented healthcare professional with a passion for helping patients navigate the healthcare system. The Case Management RN will be responsible for providing high-quality patient care and excellent customer service.The successful candidate must hold a current RN license in the state of practice, with a minimum of two years of recent OBGYN experience in a hospital setting. A strong background in case management, utilization management and/or discharge planning is preferred. Other requirements include excellent organizational and communication skills, as well as the ability to work well independently and as part of a team. Working knowledge of relevant computer applications and programs, including EMRs, is also highly desired.

Responsibilities:

  1. Provide high-quality patient care and excellent customer service to OBGYN patients.
  2. Coordinate and implement case management strategies to ensure the most efficient and effective use of resources.
  3. Develop and maintain collaborative relationships with other healthcare professionals to coordinate patient care.
  4. Monitor patient progress and adjust treatment plans as needed.
  5. Assist patients in navigating the healthcare system and understanding their benefits.
  6. Document all patient interactions in Electronic Medical Records (EMR) and other relevant computer applications.
  7. Stay up to date on changes in healthcare regulations and policies.
  8. Provide guidance and education to patients and their families on healthcare options and treatments.
  9. Assist with discharge planning, utilization management, and patient follow-up.
  10. Maintain patient confidentiality in accordance with HIPAA regulations.
