The successful candidate will have the knowledge, insight, and interpersonal skills necessary to successfully manage a wide range of complex psychiatric cases.This is an excellent opportunity for a compassionate and dedicated professional to make a difference in the lives of our patients. The ideal candidate will possess a Master’s degree in Social Work, Mental Health Counseling, or a related field, as well as at least two years of experience in case management or a related position. Responsibilities: -Provide comprehensive care management services to diverse patient populations -Develop and articulate a comprehensive plan for each patient’s care -Monitor patient progress and modify treatments as needed -Assess patient needs and provide referrals to community resources and other professionals -Maintain patient records and document all medical care and services -Maintain strict confidentiality in accordance with HIPAA regulations -Educate patients and families about diagnosis, treatments, and available resources -Collaborate with other healthcare professionals to coordinate patient care -Participate in regularly scheduled case reviews -Recognize and respond to crisis situations -Adhere to ethical and legal standards of practice in accordance with Johns Hopkins Bayview Medical Center
