
Psychiatric Case Manager
Johns Hopkins Bayview Medical Center is seeking an experienced Psychiatric Case Manager who will provide comprehensive care management services to our diverse patient population. The successful candidate will have the knowledge, insight, and interpersonal skills necessary to successfully manage a wide range of complex psychiatric cases.This is an excellent opportunity for a compassionate and dedicated professional to make a difference in the lives of our patients. The ideal candidate will possess a Master’s degree in Social Work, Mental Health Counseling, or a related field, as well as at least two years of experience in case management or a related position. The ability to empathize and collaborate with other professionals is a must. Responsibilities: -Provide comprehensive care management services to diverse patient populations -Develop and articulate a comprehensive plan for each patient’s care -Monitor patient progress and modify treatments as needed -Assess patient needs and provide referrals to community resources and other professionals -Maintain patient records and document all medical care and services -Maintain strict confidentiality in accordance with HIPAA regulations -Educate patients and families about diagnosis, treatments, and available resources -Collaborate with other healthcare professionals to coordinate patient care -Participate in regularly scheduled case reviews -Recognize and respond to crisis situations -Adhere to ethical and legal standards of practice in accordance with Johns Hopkins Bayview Medical Center
Bachelor's Degree In Social Work Or Psychology
