Case Manager

We are looking for an experienced Case Manager to join the team at Johns Hopkins Bayview Medical Center. The successful candidate will be an organized professional with excellent communication skills and an exceptional ability to build relationships with patients. The Case Manager will be responsible for coordinating patient care and providing support services to help patients achieve their health goals.The ideal candidate should have a Bachelor’s degree in a related field, such as social work, nursing, health care administration, or psychology, and be licensed in the state of Maryland (or be able to obtain the license). An understanding of the medical system, insurance regulations, and clinical documentation is also required.In this role, you will be working with a diverse population of patients, so strong interpersonal skills, compassion, and empathy are essential. You must be able to work collaboratively with other healthcare professionals and be comfortable working in a fast-paced environment.
