  1. Provide personalized care and support to Humana's members over the phone.
  2. Utilize strong clinical knowledge and skills to assess and address members' healthcare needs.
  3. Communicate effectively and compassionately with members to build rapport and trust.
  4. Collaborate with other members of the healthcare team to develop and implement care plans.
  5. Monitor and track members' health progress and provide appropriate interventions as needed.
  6. Educate members on health-related topics and encourage healthy behaviors.
  7. Maintain accurate and detailed documentation of all interactions and interventions.
  8. Adhere to all regulatory and ethical standards of care.
  9. Continuously assess and improve processes and procedures to ensure high-quality care.
  10. Demonstrate a high level of professionalism and positive attitude in all interactions.
  11. Participate in ongoing training and development to enhance skills and knowledge.
  12. Serve as an advocate for members and their healthcare needs.
  13. Maintain confidentiality and adhere to HIPAA regulations.
  14. Represent Humana in a positive and professional manner at all times.
  15. Contribute to a positive and collaborative work environment within the team.

Job Qualifications
  • Valid Nursing License: The Candidate Must Possess A Current And Valid Nursing License In The State Where They Will Be Practicing.

  • Minimum Experience: A Minimum Of 3-5 Years Of Experience In A Clinical Setting Is Required, With A Preference For Experience In Telephonic Nursing Or Care Management.

  • Case Management Certification: Candidates Should Have A Case Management Certification (Ccm) Or Be Willing To Obtain One Within Six Months Of Hire.

  • Strong Communication Skills: Excellent Verbal And Written Communication Skills Are Essential For This Role, As The Nurse Care Manager Will Be Communicating With Patients, Healthcare Providers, And Other Stakeholders Over The Phone.

  • Knowledge Of Health Insurance Industry: A Thorough Understanding Of The Health Insurance Industry, Including Medicare And Medicaid, Is Preferred. This Includes Knowledge Of Healthcare Regulations, Policies, And Procedures.
