  1. Provide compassionate and dedicated social work services to patients and their families.
  2. Conduct thorough assessments of patients' social, emotional, and psychological needs.
  3. Develop individualized care plans in collaboration with patients, families, and healthcare team.
  4. Connect patients and families with appropriate community resources and support services.
  5. Advocate for patients' rights and needs within the healthcare system.
  6. Provide education and counseling to patients and families on coping strategies and resources.
  7. Collaborate with interdisciplinary team to ensure comprehensive and coordinated care for patients.
  8. Maintain accurate and timely documentation of patient interactions and progress.
  9. Stay current on social work best practices and healthcare regulations.
  10. Demonstrate strong communication and interpersonal skills when interacting with patients, families, and team members.
  11. Work independently and efficiently in a fast-paced and dynamic environment.
  12. Adhere to ethical and professional standards of social work practice.
  13. Attend team meetings and participate in continuous quality improvement initiatives.
  14. Demonstrate empathy, understanding, and respect for diverse backgrounds and cultures.
  15. Maintain confidentiality and adhere to HIPAA regulations in all aspects of the job.

Job Qualifications
  • Bachelor's Degree In Social Work Or A Related Field: A Minimum Of A Bachelor's Degree In Social Work From An Accredited Institution Is Often Required For A Social Worker Position At Humana. This Degree Provides The Necessary Knowledge And Skills To Effectively Work With Individuals And Families In A Home Health Setting.

  • State Licensure And/Or Certification: Depending On The State In Which The Job Is Located, A Social Worker At Humana May Need To Hold A State License Or Certification. These Requirements Ensure That The Individual Meets The Professional Standards And Regulations For Practicing Social Work.

  • Experience In Home Health Or Medical Social Work: Previous Experience Working In A Home Health Setting Or In Medical Social Work Is Highly Preferred For This Position. This Experience Allows The Social Worker To Understand The Unique Challenges And Needs Of Clients Receiving Home Health Care.

  • Strong Communication And Interpersonal Skills: As A Social Worker At Humana, Effective Communication And Interpersonal Skills Are Essential. This Includes The Ability To Build Rapport And Trust With Clients And Their Families, As Well As Collaborate With Other Healthcare Professionals.

  • Knowledge Of Community Resources And Support Services: Humana Values Social Workers Who Have A Strong Understanding Of Local Community Resources And Support Services. This Knowledge Allows The Social Worker To Connect Clients With The Resources They Need To Improve Their Overall Well-Being And Quality Of Life.
