1. Provide compassionate and high-quality care to patients in their homes.
2. Utilize nursing skills and knowledge to assess, plan, implement, and evaluate patient care.
3. Collaborate with interdisciplinary team members to develop and implement individualized care plans.
4. Administer medications, treatments, and procedures as prescribed by the physician.
5. Monitor patient progress and communicate any changes or concerns to the physician and other team members.
6. Educate patients and their families on disease management, self-care, and treatment plans.
7. Maintain accurate and complete patient records and documentation.
8. Adhere to all state and federal regulations and company policies and procedures.
9. Participate in quality improvement initiatives and ensure compliance with standards of care.
10. Serve as an advocate for patients and their families, ensuring their needs and preferences are addressed.
11. Communicate effectively with patients, families, and team members to promote a positive and collaborative work environment.
12. Continuously update knowledge and skills through ongoing education and professional development opportunities.
