  1. Utilize clinical expertise to provide top-quality, personalized care to Humana members.
  2. Serve as a guide and support system for members throughout their health care journey.
  3. Communicate effectively with members to understand their needs and provide appropriate resources and solutions.
  4. Collaborate with interdisciplinary teams to coordinate and manage members' care plans.
  5. Conduct assessments and develop care plans that meet members' physical, emotional, and social needs.
  6. Monitor and report on members' progress and make necessary adjustments to care plans.
  7. Educate members on their health conditions and treatment options.
  8. Advocate for members' needs and preferences within the healthcare system.
  9. Ensure timely and accurate documentation of member interactions and care plans.
  10. Stay updated on industry trends and best practices to continually improve the quality of care provided.
  11. Maintain a high level of compassion and empathy towards members.
  12. Work closely with other healthcare professionals, including doctors and specialists, to coordinate and deliver comprehensive care.
  13. Participate in quality improvement initiatives to enhance the overall member experience.
  14. Adhere to all ethical and legal standards in patient care.
  15. Demonstrate a strong commitment to teamwork and collaboration with colleagues.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field

  • Current Rn License In Good Standing

  • Minimum Of 3-5 Years Of Clinical Experience As A Registered Nurse

  • Knowledge And Understanding Of Healthcare Systems, Including Medicare And Medicaid

  • Strong Communication And Interpersonal Skills, With The Ability To Build Rapport With Patients And Providers
