  1. Provide telephonic support and care coordination for Humana members.
  2. Utilize strong clinical skills to assess and address members' healthcare needs.
  3. Communicate effectively with members, caregivers, and healthcare providers to coordinate care plans.
  4. Collaborate with interdisciplinary team to develop and implement individualized care plans.
  5. Monitor and evaluate members' health status and make appropriate recommendations for care.
  6. Educate members on disease management and preventative care strategies.
  7. Maintain accurate and timely documentation of all member interactions and care plans.
  8. Adhere to all regulatory and compliance guidelines.
  9. Continuously strive to improve quality of care and member satisfaction.
  10. Serve as an advocate for members and ensure their needs are met.
  11. Keep up-to-date with industry knowledge and best practices in telephonic nursing.
  12. Participate in team meetings and contribute to the development of policies and procedures.
  13. Collaborate with other departments to ensure seamless coordination of care.
  14. Demonstrate empathy and compassion towards members and their families.
  15. Represent Humana in a professional and positive manner at all times.

Job Qualifications
  • Active Rn Licensure: The Candidate Must Possess An Active And Unrestricted Rn License In The State Where They Will Be Working As A Telephonic Nurse For Humana.

  • Clinical Experience: A Minimum Of 3-5 Years Of Clinical Experience As A Registered Nurse Is Required For This Role, Preferably In A Care Management Or Telephonic Nursing Setting.

  • Care Management Certification: The Candidate Should Hold A Valid Certification In Care Management, Such As Certified Case Manager (Ccm) Or Accredited Case Manager (Acm), To Demonstrate Their Expertise In The Field.

  • Strong Communication Skills: As A Telephonic Nurse, The Candidate Will Be Responsible For Communicating With Members, Providers, And Other Healthcare Professionals On A Daily Basis. Therefore, Excellent Verbal And Written Communication Skills Are Essential.

  • Knowledge Of Medicare And Medicaid Regulations: Humana Is A Healthcare Company That Primarily Serves Medicare And Medicaid Populations. Therefore, The Candidate Must Have A Thorough Understanding Of These Programs, Their Regulations, And Reimbursement Guidelines.
