- Conduct thorough assessments of patients' health status and develop personalized care plans.
- Administer medications, treatments, and other nursing interventions as prescribed by the physician.
- Monitor patients' progress and make necessary adjustments to the care plan.
- Educate patients and their families about their health conditions, treatment plans, and self-care techniques.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Document all patient care activities accurately and timely.
- Advocate for patients' rights and ensure their needs are met.
- Adhere to all state and federal regulations and Humana's policies and procedures.
- Participate in ongoing education and training to continuously enhance nursing skills and knowledge.
- Maintain confidentiality and professionalism at all times.

Current Rn Licensure: A Qualified Candidate For The Registered Nurse Case Manager Home Health Job At Humana Must Hold A Current And Valid Rn License In The State In Which They Will Be Practicing.
Minimum Of 2 Years Of Home Health Experience: Humana Requires A Minimum Of 2 Years Of Experience In Home Health Nursing In Order To Be Considered For The Registered Nurse Case Manager Home Health Position. This Experience Should Include Managing Patient Care And Coordinating With Other Healthcare Providers.
Strong Clinical Assessment Skills: The Ideal Candidate Will Possess Strong Clinical Assessment Skills In Order To Accurately Assess And Evaluate The Needs Of Patients In A Home Health Setting. This Includes The Ability To Identify And Address Potential Barriers To Care.
Excellent Communication And Interpersonal Skills: As A Case Manager, Strong Communication And Interpersonal Skills Are Crucial To Effectively Collaborate With Patients, Their Families, And Other Healthcare Providers. The Candidate Should Be Able To Communicate Clearly And Compassionately With Diverse Populations.
Familiarity With Medicare And Medicaid Regulations: Humana Is A Leading Provider Of Medicare Advantage And Medicaid Managed Care Plans. Therefore, Candidates With Knowledge And Understanding Of Medicare And Medicaid Regulations And Guidelines Will Be Preferred For This Role. This Includes Experience With Documentation And Billing Procedures Specific To These Programs.
