Job Description

Humana is seeking a Per Diem Physical Therapist for our Home Health team. In this role, you will have the opportunity to provide personalized and effective care to patients in the comfort of their own homes.   1. Provide personalized and effective physical therapy services to patients in their homes.
  2. Conduct thorough assessments and develop individualized treatment plans for patients.
  3. Administer therapeutic exercises, manual therapy, and other modalities as needed.
  4. Regularly monitor and document patient progress and adjust treatment plans accordingly.
  5. Communicate and collaborate with other healthcare professionals involved in the patient's care.
  6. Educate patients and their families on proper techniques and exercises for continued improvement.
  7. Maintain accurate and up-to-date patient records.
  8. Adhere to all professional, ethical, and legal standards in the provision of care.
  9. Continuously stay updated on current physical therapy practices and techniques.
  10. Maintain a positive and compassionate attitude towards patients and their families.
  11. Participate in team meetings and contribute to the development of the home health program.
  12. Provide exceptional customer service and address any patient concerns or questions.
  13. Ensure compliance with company policies and procedures.
  14. Adhere to all safety and infection control protocols.
  15. Flexibility to work on a per diem basis to accommodate patient needs and company's schedule.

Job Qualifications
  • Bachelor's Degree In Physical Therapy: A Minimum Of A Bachelor's Degree In Physical Therapy From An Accredited Institution Is Required For This Position. This Demonstrates A Strong Foundation Of Knowledge And Skills In The Field Of Physical Therapy.

  • Current State Licensure: Candidates Must Possess A Current And Active License To Practice Physical Therapy In The State Where The Job Is Located. This Ensures That The Candidate Has Met All Necessary Requirements And Is Legally Qualified To Provide Physical Therapy Services.

  • Previous Home Health Experience: Experience Working In A Home Health Setting Is Highly Preferred For This Position. This Demonstrates The Ability To Provide Care In A Non-Traditional Setting And A Strong Understanding Of The Unique Challenges And Considerations Of Home Health Care.

  • Strong Communication And Interpersonal Skills: As A Home Health Physical Therapist, Effective Communication With Patients, Their Families, And Other Healthcare Professionals Is Crucial. Candidates Should Possess Strong Communication Skills And The Ability To Build Positive Relationships With Others.

  • Time Management And Organizational Skills: In A Per Diem Role, Physical Therapists Must Be Able To Manage Their Time Effectively And Prioritize Tasks To Ensure Timely And Efficient Delivery Of Care. Strong Organizational Skills Are Also Important For Keeping Track Of Patient Schedules, Documentation, And Other Administrative Tasks.
