  1. Provide high-quality physical therapy services to Humana members in their homes.
  2. Conduct initial evaluations and develop personalized treatment plans for each member.
  3. Implement and monitor treatment plans, making adjustments as necessary.
  4. Utilize evidence-based practices and stay current with industry developments and best practices.
  5. Maintain accurate and timely documentation of all treatments and progress.
  6. Collaborate with other healthcare professionals, including physicians and nurses, to ensure coordinated and comprehensive care for members.
  7. Communicate effectively with members, their families, and caregivers to educate them on treatment plans and goals.
  8. Provide compassionate and patient-centered care, taking into consideration the unique needs and preferences of each member.
  9. Adhere to all ethical and legal standards in the practice of physical therapy.
  10. Participate in ongoing training and development opportunities to enhance skills and knowledge.
  11. Maintain a positive and professional attitude while representing Humana.
  12. Comply with all company policies and procedures.
  13. Continuously monitor and assess the member's progress and adjust treatment plans accordingly.
  14. Serve as an advocate for members and their healthcare needs.
  15. Uphold a high level of customer service and satisfaction.
  16. Adhere to the company's quality assurance and performance improvement programs.
  17. Maintain a safe and clean working environment.
  18. Demonstrate a commitment to personal and professional growth.
  19. Participate in team meetings and contribute to the development of new initiatives and strategies.
  20. Follow all HIPAA guidelines and maintain confidentiality of member information.

Job Qualifications
  • Bachelor's Or Master's Degree In Physical Therapy From An Accredited University.

  • Valid State License To Practice As A Physical Therapist.

  • Minimum Of 2 Years Of Experience In Home Health Care Setting.

  • Strong Understanding Of Medicare And Medicaid Regulations For Home Health Therapy Services.

  • Excellent Communication And Interpersonal Skills To Work With Patients, Family Members, And Other Healthcare Professionals.
