  1. Provide exceptional support and coordination for home health patients and their families.
  2. Communicate effectively with patients, their families, and healthcare providers to ensure quality care.
  3. Coordinate and schedule appointments, home visits, and follow-up care.
  4. Maintain accurate and up-to-date patient records and documentation.
  5. Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  6. Serve as a liaison between patients, families, and healthcare providers.
  7. Assist in coordinating necessary medical equipment and supplies for patients.
  8. Educate patients and families on available resources and services.
  9. Ensure compliance with all regulatory and company policies and procedures.
  10. Continuously monitor and evaluate the effectiveness of patient care plans.
  11. Provide excellent customer service and address any patient concerns or issues.
  12. Participate in team meetings and contribute to the improvement of processes and procedures.
  13. Stay updated on industry developments and trends to enhance patient care.
  14. Maintain confidentiality of patient information.
  15. Adhere to ethical and professional standards in all interactions with patients and colleagues.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Business Administration, Or Related Field.

  • Minimum Of 2 Years Of Experience In A Patient Services Coordinator Or Similar Role, Preferably In Home Health.

  • Knowledge Of Federal And State Regulations For Home Health Services, As Well As Insurance And Reimbursement Policies.

  • Excellent Communication And Interpersonal Skills To Effectively Interact With Patients, Caregivers, And Healthcare Professionals.

  • Proficiency In Electronic Medical Records And Scheduling Software, As Well As Strong Organizational And Time-Management Skills.
