1. Provide compassionate and dedicated care to Humana members as a Medical Social Worker within the Home Health team.
2. Communicate effectively and build rapport with members to understand their needs and concerns.
3. Conduct comprehensive assessments to identify social, emotional, and environmental factors that may impact members' health.
4. Develop individualized care plans based on members' needs and goals.
5. Collaborate with other health care professionals to coordinate and implement care plans.
6. Connect members with community resources and support services to improve their overall well-being.
7. Advocate for members' rights and ensure they have access to necessary healthcare services.
8. Monitor and evaluate members' progress and provide ongoing support and counseling as needed.
9. Maintain accurate and timely documentation of all interactions and interventions with members.
10. Stay up-to-date with industry developments and best practices in medical social work.
11. Participate in team meetings and case conferences to discuss members' progress and develop effective strategies for care.
12. Adhere to all ethical and legal standards in the provision of care to members.
13. Maintain confidentiality and protect members' privacy at all times.
14. Continuously strive to improve the quality of care and services provided to members.
15. Act as a positive representative of Humana and promote the organization's mission and values.
