  1. Provide comprehensive social work services to patients and their families in a home health care setting.
  2. Conduct thorough assessments of patients' social, emotional, and environmental needs.
  3. Develop individualized care plans in collaboration with the patient, family, and healthcare team.
  4. Assist patients and families in navigating the healthcare system and accessing community resources.

Job Qualifications
  • Valid Licensure: A Qualified Medical Social Worker Should Possess A Valid State Licensure As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw).

  • Previous Experience: Ideally, The Candidate Should Have At Least 2 Years Of Experience In A Healthcare Setting, With Specific Experience In Home Health Preferred.

  • Knowledge Of Medical Social Work Practices: The Candidate Should Have A Strong Understanding Of Medical Social Work Principles And Practices, Including Knowledge Of Community Resources, Patient Assessment, And Care Planning.

  • Excellent Communication Skills: As A Medical Social Worker, The Candidate Will Be Required To Communicate Effectively With Patients, Families, And Interdisciplinary Team Members. Strong Verbal And Written Communication Skills Are Essential.
