  1. Lead and manage a team of Care Coaches to ensure efficient and effective delivery of care coordination services to Humana members.
  2. Develop and implement strategies to enhance the quality of care coordination provided by the team.
  3. Ensure compliance with all relevant regulations and standards for care coordination and coaching.
  4. Collaborate with other departments and teams to ensure seamless coordination of care for members.
  5. Monitor and analyze data to identify areas for improvement and make necessary adjustments to processes and procedures.
  6. Provide ongoing training, coaching, and development opportunities for the Care Coaches.
  7. Foster a positive and supportive work environment that promotes teamwork and encourages personal and professional growth.
  8. Develop and maintain relationships with external healthcare providers and community resources to support the care coordination process.
  9. Participate in regular meetings and communication with leadership to provide updates on team performance and identify areas for improvement.
  10. Serve as a role model for exceptional customer service and inspire the team to deliver the same level of service to members.
  11. Stay up-to-date with industry trends and best practices in care coordination and coaching to continuously improve the team's performance.
  12. Handle any escalated issues or complaints from members, ensuring timely and satisfactory resolution.
  13. Collaborate with the recruitment team to hire and onboard new Care Coaches as needed.
  14. Maintain accurate documentation and records of team performance, member interactions, and other relevant data.
  15. Uphold the values and mission of Humana by promoting a culture of compassion, empathy, and dedication to improving the health and well-being of members.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Social Work, Or A Related Field.

  • Minimum Of 3 Years Of Experience In Care Coordination Or Case Management, Preferably Within A Healthcare Setting.

  • Strong Leadership Skills And Experience Managing A Team Of Care Coordinators Or Other Healthcare Professionals.

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Other Departments And External Stakeholders.

  • Knowledge Of Healthcare Regulations And Guidelines, Such As Hipaa And Medicare/Medicaid, As Well As Experience Using Electronic Health Record Systems.
